Pharmacokinetics in children

In adults, paracetamol is conjugated in the liver with glucuronic acid (approx. 60%), sulphate (approx. 35%) and cysteine (approx. 3%). In neonates and children up to 12 years of age, sulphate conjugation is the predominant elimination pathway and glucuronidation occurs to a lesser extent than in adults. However, the total elimination in children as a result of the increased sulphation capacity is broadly comparable to that of adults.

  • Oral
    • 6 years up to 9 years
      • Paracetamol/caffeine: 250 mg/25 mg 4-6 times daily

    • 9 years up to 12 years
      • Paracetamol/caffeine 500 mg/50 mg 3-4 times daily

    • 12 years up to 15 years
      • Paracetamol/caffeine 500 mg/50 mg 4-6 times daily

    • ≥ 15 years
      • Paracetamol/caffeine 500-1000 mg/50-100 mg, maximum 3000 mg/300 mg per day

Renal impaiment in children > 3 months

GFR ≥10 ml/min/1.73m2: Dose adjustment not required.

GFR <10 ml/min/1.73m2: A general recommendation on dose adjustment cannot be provided.
