Dosages

Familial Mediterranean fever (FMF) prophylaxis
  • Oral
    • 1 month up to 5 years
      • 0.5 mg/day in 1 - 2 doses. increase step by step if necessary (0.25 mg per step) to a maximum of 2 mg/day..
    • 5 years up to 10 years
      • 1 mg/day in 1 - 2 doses. increase step by step if necessary (0.25 mg per step) to a maximum of 2 mg/day.
    • ≥ 10 years
      • 1.5 mg/day in 1 - 2 doses. increase step by step if necessary (0.25 mg per step) to a maximum of 2 mg/day.

Renal impaiment in children > 3 months

  • GFR ≥30: dosage adjustment is not necessary.
  • GFR 10-30: 50% of normal starting dose, adjustment of dosing interval is not needed
  • GFR <10: general advice is not given.
Clinical consequences

With impaired renal function, the risk of toxicity may be increased.

Clinical Effects:
First signs of toxicity are a burning sensation in the throat, nausea, abdominal cramps, and diarrhea. Colchicine should be discontinued immediately in this case.

Toxic with severe side effects, both acute and chronic. Burning sensation in the throat, nausea, abdominal cramps and diarrhoea. Neuropathy can arise in long-term use.

Colchicine has small therapeutic window and therefore there is a risk of toxicity. Discontinue administration in case of toxic symptoms such as burning sensation in the throat, nausea, vomiting, abdominal pain and diarrhea.
