Prophylaxe und symptomatische Therapie von Übelkeit und Erbrechen unterschiedlicher Genese, insbesondere von Kinetosen:

  • Oral
    • Die Dosierung von Dimenhydrinat soll sich am Körpergewicht der Patienten und an der Bioverfügbarkeit der jeweiligen Darreichungsform orientieren. Insbesondere bei jungen oder leichten Patienten kann schon die niedrigste Dosierung in der jeweils zutreffenden Gewichts- oder Altersklasse zur Linderung der Symptome ausreichend sein. Dabei sollte im Allgemeinen eine Dosierung von 5 mg/kg Körpergewicht (KG) pro Tag nicht überschritten werden. [10]
    • Kleinkinder ab 6 kg Körpergewicht: 3–4 mal täglich 1,25 mg Dimenhydrinat/kg Körpergewicht [1]
      • 6 bis 10 kg: Einzeldosis 8,25 mg, Tagesgesamtdosis 24,75 mg
      • >10 bis 15 kg: Einzeldosis 16,5 mg, Tagesgesamtdosis 49,5 mg
      • >15 bis 20 kg: Einzeldosis 24,75 mg, Tagesgesamtdosis 74,25 mg
      • >20 bis 25 kg: Einzeldosis 24,75 mg, Tagesgesamtdosis 74,25-99 mg
      • >25 bis 30 kg: Einzeldosis 41,25 mg, Tagesgesamtdosis 124 mg
    • Kinder >30 kg Körpergewicht: Können erforderlichenfalls 3 mal täglich 50 mg Dimenhydrinat pro Einzelgabe, entsprechend einer Tagesgesamtmenge von 150 mg Dimenhydrinat erhalten. [1]
    • Kinder ab 6 bis 12 Jahre: 5 mg Dimenhydrinat/kg Körpergewicht, unterteilt in vier Einzeldosen oder 25–50 mg alle 6–8 Stunden, jedoch nicht mehr als 150 mg täglich[6]. Bei leichten Fällen der Reisekrankheit 2-mal je 20 mg Dimenhydrinat (wirkstoffhaltiges Kaugummi) im Abstand von jeweils ½ Stunde, jedoch nicht mehr als 80 mg Dimenhydrinat am Tag. [12]
    • Kinder im Alter >12 Jahre / Erwachsene: Die übliche Dosis von Dimenhydrinat beträgt 50 mg bis 100 mg, 3 oder 4 Mal täglich eingenommen. Die Tagesdosis sollte 400 mg Dimenhydrinat nicht überschreiten [7]. Bei leichten Fällen der Reisekrankheit 3-mal je 20 mg Dimenhydrinat (wirkstoffhaltiges Kaugummi) im Abstand von jeweils ½ Stunde, jedoch nicht mehr als 140 mg Dimenhydrinat am Tag. [12]
  • Oral, retard
    • Kinder unter 14 Jahre: Dürfen bei Kindern unter 14 Jahre nicht angewendet werden. Hierzu stehen spezielle niedriger dosierte Darreichungsformen zur Verfügung. [2]
    • Erwachsene und Jugendliche über 14 Jahre: Bei Erwachsenen und Jugendlichen über 14 Jahren beträgt die Dosierung 200 bis maximal 400 mg Dimenhydrinat pro Tag, verteilt auf mehrere Einzeldosen. Im Allgemeinen morgens und abends je 150 mg, retardiert (entsprechend 300 mg Dimenhydrinat pro Tag), jedoch nicht mehr als 400 mg pro Tag. Im Ausnahmefall können die Einzelgaben bei unzureichender Wirkung im Abstand von 8 bis 10 Stunden erfolgen. [2]
  • Intravenös
    • Kinder ab 6 kg Körpergewicht: 1,25 mg/kg Körpergewicht, 1–3 mal täglich [4, 8]
    • Kinder von 6 bis 14 Jahre: 25–50 mg 1–3 mal täglich. Eine Tageshöchstdosis von 150 mg sollte nicht überschritten werden. [4]
    • Erwachsene und Jugendliche über 14 Jahre:
      • Intravenös: 62-186 mg pro Tag [4]
      • Eine Tageshöchstdosis von 400 mg sollte nicht überschritten werden. [4]
  • Intramuskulär
    • Kinder unter 3 Jahre: Die Anwendung bei Kindern unter 3 Jahren ist kontraindiziert [13]
    • Kinder ab 6 kg Körpergewicht: 1,25 mg/kg Körpergewicht, 1–3 mal täglich [4, 8]
    • Kinder von 6 bis 14 Jahre: 25–50 mg 1–3 mal täglich. Eine Tageshöchstdosis von 150 mg sollte nicht überschritten werden. [8]
    • Erwachsene und Jugendliche über 14 Jahre:
      • 100–300 mg pro Tag [8]
      • Eine Tageshöchstdosis von 400 mg sollte nicht überschritten werden. [8]
  • Rektal
    • Die Dosierung von Dimenhydrinat soll sich am Körpergewicht der Patienten und an der Bioverfügbarkeit der jeweiligen Darreichungsform orientieren.     • Jugendliche über 14 Jahre und Erwachsene: 150–300 mg pro Tag, in 1–2 Dosen [9]
    • Jugendliche und Erwachsene mit über 14 Jahre: 1–2 x täglich 150 mg, maximal 400 mg Dimenhydrinat/Tag. Sollte die Wirkung bei dieser Dosierung nicht ausreichend sein, können Erwachsene und Jugendliche, die mehr als 70 kg wiegen, erforderlichenfalls bis zu 3 × täglich 150 mg erhalten. [9]

Schwindel

  • Oral
    • Kinder bis zu 6 Jahren: Für Kinder bis zu 6 Jahren stehen 40 mg Zäpfchen zur Verfügung. [10]
    • Kinder von 6 bis 14 Jahre: 1- bis 3-mal täglich 25–50 mg Dimenhydrinat (entsprechend 25–150 mg Dimenhydrinat täglich), jedoch nicht mehr als 150 mg Dimenhydrinat pro Tag. Im Allgemeinen sollte eine Dosierung von 5 mg/kg KG pro Tag nicht überschritten werden. Das bedeutet, dass Kinder unter 30 kg im Allgemeinen nicht mehr als 100 mg pro Tag erhalten. [10]
    • Erwachsene und Jugendliche über 14 Jahre: 1- bis 4-mal täglich 1-2 Tabletten Dimenhydrinat (entsprechend 50-400 mg Dimenhydrinat täglich), jedoch nicht mehr als 400 mg Dimenhydrinat pro Tag. [10]
  • Oral, retard
    • Kinder unter 14 Jahren: Dürfen nicht bei Kindern unter 14 Jahren angewendet werden. [3]
    • Ältere Patienten: Ein- bis zweimal täglich 120 mg, retardiert. Innerhalb von 24 Stunden können – möglichst im 8-Stunden-Abstand – bis zu drei Dosen Dimenhydrinat eingenommen werden, jedoch nicht mehr als 400 mg pro Tag. [3]
  • Rektal
    • Kleinkinder mit 8 bis < 16 kg KG: 1-mal täglich 1 Zäpfchen Dimenhydrinat 40 mg (entsprechend 40 mg Dimenhydrinat pro Tag) [5]
    • Kinder mit 15 bis 25 kg KG: 1-mal täglich 1 Zäpfchen Dimenhydrinat 70 mg (entsprechend 70 mg Dimenhydrinat pro Tag) [5]
    • Kinder ab 16 bis 25 kg KG: 2-mal täglich 1 Zäpfchen Dimenhydrinat 40 mg (entsprechend 80 mg Dimenhydrinat pro Tag) [5]
    • Kinder über 25 kg KG: 2- bis 3-mal täglich 1 Zäpfchen Dimenhydrinat 40 mg (entsprechend 80-120 mg Dimenhydrinat pro Tag) [5]
    • Kinder mit 25 bis 40 kg KG: 2-mal täglich 1 Zäpfchen Dimenhydrinat 70 mg (entsprechend 140 mg Dimenhydrinat pro Tag) [5]
    • Kinder ab 40 kg KG und Jugendliche ab 14 Jahre: 2- bis 3-mal täglich 1 Zäpfchen Dimenhydrinat 70 mg (entsprechend 140-210 mg Dimenhydrinat pro Tag) [5]
    • Überdosierung mit Dimenhydrinat kann insbesondere bei Kindern unter 3 Jahren lebensbedrohlich sein und muss deshalb insbesondere in dieser Altersgruppe unter allen Umständen vermieden werden. Es dürfen nicht mehr als 5 mg/kg KG/24 Stunden gegeben werden. Zudem sollte folgende Obergrenze eingehalten werden: Kinder von 6 bis 14 Jahre maximal 150 mg Dimenhydrinat/Tag. Erwachsene und Jugendliche über 14 Jahre maximal 400 mg Dimenhydrinat/Tag. [5]

Dimenhydrinat ist in Form von Tabletten, überzogenen Tabletten, Retardkapseln, Kautabletten/-gummi, Sublingualtabletten, Sirupen, Injektionslösungen und Suppositorien verfügbar.

Der Wirkstoff liegt stets als Reinform vor, worauf sich auch die Dosisangabe bezieht.

Orale Anwendung

Anwendungshinweis:

Zur Prophylaxe von Kinetosen erfolgt die erstmalige Gabe ca. ½–1 Stunde vor Reisebeginn. Zur Therapie von Übelkeit und Erbrechen werden die Gaben in regelmäßigen Abständen über den Tag verteilt. [11]

      • Treatment of banal gastroenteritis: Studies have shown that the administration of dimenhydrinate to infants and young children for the treatment of banal gastroenteritis shows no benefit compared to substitution with fluids and electrolytes alone. Against the background of the increased occurrence of serious side effects in children up to three years of age in this indication, dimenhydrinate should not be used for the treatment of banal gastroenteritis.

Warnings & precautions in children

Studies have shown that administrating dimenhydrinate to infants and young children for the treatment of banal gastroenteritis has no benefit compared with substitution with liquid and electrolytes alone. In view of the frequent occurrence of serious side effects in children up to three years of age in this indication, dimenhydrinate should not be used for the treatment of banal gastroenteritis in this agegroup.

Since dimenhydrinate can cause a seizure, particularly in small children, and this group of patients also tends to have febrile seizures, dimenhydrinate should not be given for febrile infections.

Only use if clearly indicated in children up to 3 years. The recommended maximum dose for this age group should never be exceeded.
