Side effects in children
In children aged 6 years and under, the risk of side effects such as coughing and laryngeal spasms is elevated. This occurs particularly when the laryngeal mask is removed under deep anaesthesia.
Postoperative agitation has been reported in children that can be expressed as disorientation, tearful behaviour and striking movements.

Contra-indications in children
For the induction of inhalation anesthesia in children, since coughing, breath-holding, respiratory arrest, laryngospasm and increased secretions often occur.
Due to the limited data available in non-intubated paediatric patients, desflurane is not indicated for the maintenance of anaesthesia in non-intubated children. (SmPC Suprane)

Warnings & precautions in children
Use in children with bronchial hyperresponsiveness:
Desflurane should definitely be used with extra caution in children with asthma or recent upper respiratory tract infections due to the risk of airway narrowing and increased airway resistance.
Maintenance of anaesthesia in children:
Caution should be exercised when using desflurane for maintenance of anaesthesia with a laryngeal mask airway (LMA), particularly in children up to 6 years of age, as there is an increased risk of respiratory complications such as cough and laryngospasm, especially after discontinuation of the laryngeal mask airway under deep anaesthesia.
When children recover after anaesthesia, a brief period of agitation may occur that prevents cooperation.

Middle ear operations:
Desflurane, like other volatile anaesthetics, leads to an increase in pressure in the middle ear, especially in children. Monitoring of the pressure in the middle ear is therefore recommended during anaesthesia with desflurane.
