Photophobia, eye irritation, and blurred vision are the most common side effects (SmPC Ryjunea).
Systemic side effects rarely occur but include anticholinergic effects such as dry mouth and skin, flushing, increased body temperature, urinary symptoms, gastrointestinal symptoms and tachycardia. These effects are more likely to occur in infants and children.
Infants may very rarely experience life-threatening conditions such as drowsiness, cramps, high fever and coma.

Children over 3 months to 1.5 years of age should receive atropine eye drops at a concentration of 0.5%.

Pressing the tear duct closed briefly during and immediately after administration reduces the risk of systemic reactions. Care should be taken to ensure that the drop solution does not flow quickly into the nose and throat. (SmPC Ryjunea)
As the ability to regulate temperature may be impaired by inhibition of sweat secretion, caution is required in feverish patients and especially in children.
Infants and toddlers up to the age of two are particularly sensitive to systemic effects, as are patients with Down syndrom and children with spastic paralysis. Particularly careful dosage is therefore necessary in these cases.
