Adult Side Effects
Meest voorkomend hypoglykemie. Vaak (1–10%): lokale overgevoeligheidsreacties, zoals roodheid, jeuk en zwelling (meestal voorbijgaand na enkele dagen tot weken). Soms (0,1–1%): lokale lipodystrofie, vooral bij herhaalde injectie op dezelfde plaats. Zelden (0,1–0,01%): gegeneraliseerde allergische reacties, variërend van urticaria, erupties of gastro-intestinale klachten tot levensbedreigende anafylactische shock met o.a. bronchospasme, larynxoedeem en circulatoire collaps.

Bij kinderen alleen toepassen indien er een te verwachten voordeel is in vergelijking met oplosbare insulinen (zoals het tijdstip van injecteren in relatie tot de maaltijd).

Alerts And Precautions Adults
Bij nier- of leverinsufficiëntie de bloedglucosespiegel vaker controleren. Verandering van insulinebehoefte treedt op bij verandering van het maaltijdschema, (onverwachte) zware fysieke inspanning, het ontstaan van stress–situaties zoals ziekten (bv. griep) en emotionele gebeurtenissen, en verder bij bijkomende aandoeningen die de werking van nieren, lever, bijnier, hypofyse of de schildklier beïnvloeden. Hypoglykemie treedt mogelijk eerder op bij snelwerkende insulineanalogen dan bij oplosbare humane insuline. Bij een sterke verbetering van de bloedglucose-instelling door een intensieve insulinetherapie, bij overschakeling op een ander type insuline, bij een lange historie van diabetes of een diabetische zenuwaandoening en bij ouderen kunnen de vroege waarschuwingssignalen van hypoglykemie anders worden waargenomen of zelfs verdwijnen. Bij normale of verlaagde waarden van geglycosyleerd hemoglobine denken aan terugkerende, niet-onderkende (vooral nachtelijke) episoden van hypoglykemie. Intensiveer – vanwege de kans op ernstige complicaties – de controle op hypoglykemie bij een significante stenose van coronaire arteriën of van de bloedvaten die de hersenen van bloed voorzien en bij (onbehandelde) proliferatieve retinopathie. Een abrupte verbetering van de bloedglucose-instelling (door intensieve insulinetherapie) kan gepaard gaan met tijdelijke verergering van diabetische retinopathie. Bij inadequate dosering of onderbreken van de behandeling kan vooral bij type 1-diabeten hyperglykemie optreden, met als symptomen dorst, frequente mictie, misselijkheid, braken, sufheid, een rode, droge huid, droge mond en anorexie. Onbehandelde hyperglykemie kan bij type 1-diabetes tot diabetische ketoacidose leiden (adem ruikt naar aceton).
