    • ≥ 6 years and ≥ 20 kg
      • 0.15 mg/kg/day in 1 dose
        • Due to large differences in sensitivity, start with the lowest possible dose and slowly increase to optimal efficacy and tolerability.
        • max: 6 mg/day (outpatient); 12 mg/day (in hospital)
