Dosages

Status epilepticus
  • Intravenous
    • 1 month up to 18 years
      • Initial dose: 5 mg/kg/dose, once only.
      • Maintenance dose: 0.5 - 5 mg/kg/hour, continuous infusion.
        • Loading: Preferably with EEG monitoring. Repeat the loading dose every 20 minutes until a burst suppression of 30 seconds is obtained or the epileptiform activity has disappeared.
        • Maintenance: raise the dosage as necessary in steps of 1-2 mg/kg/hour to a maximum of 10 mg/kg/hour.
        • Reduce: Once the status is under control (both clinically and on the EEG), cut back the pentobarbital gradually over 12-24 hours (e.g. 25% of the initial rate every 6 hours)
        • Relapses: repeat the loading dose and revert to the old infusion rate for 24-72 hours.
Agitation in terminal illness
  • Intravenous
    • 1 month up to 18 years
      • 0.5 - 1 mg/kg/dose every 5 min until sufficient effect is achieved.

Renal impaiment in children > 3 months

GFR ≥10 ml/min/1.73m2: Dose adjustment not required.

GFR <10 ml/min/1.73m2: A general recommendation on dose adjustment cannot be provided.
