Dosages

Prophylaxis of deep venous thrombosis in heparin-induced thrombocytopenia
  • Subcutaneous
    • < 2 years
      • 8 - 144 AntiXa/kg/day in 2 - 3 doses.
      • Target plasma anti-Xa activity level: 0.1-0.4 IU/ml

    • 2 years up to 7 years

    • 7 years up to 18 years
      • 20 - 25 AntiXa/kg/day in 2 - 3 doses. Max: 1.500 AntiXa/day.
      • Target plasma anti-Xa activity level: 0.1-0.4 IU/ml

Treatment of deep venous trombosis in heparin-induced thrombocytopenia (HIT)
  • Intravenous
    • 7 months up to 18 years
      • Initial dose: 30 AntiXa/kg/dose, once only.
      • Maintenance dose: 1.2 - 2 AntiXa/kg/hour, continuous infusion.
      • Duration of treatment:

        Daily check of the anti-Xa level (target value: 0.4-0.8 IU/ml)

Danaparoid is excreted primarily with the urine; accumulation of danaparoid can be expected to occur with decreased renal function. This increases the risk of bleeding.
