  • Oral
    • 1 month up to 18 years and < 30 kg
      • 50 mg/kg/day in 2 doses. Max: 1.500 mg/day.
    • 1 month up to 18 years and ≥ 30 kg
      • 750 - 1.500 mg/day in 2 doses.

Renal impaiment in children > 3 months

  < 30 kg ≥ 30 kg
≥ 15 mL/min/1,73m2 no dose adjustment needed
<15 mL/min/1,73m2 25 mg/kg/day in 1 dose 375-750 mg/day in 1 dose
