Dosages

Metabolic alkalosis
  • Intravenous
    • 1 month up to 18 years
      • Initial dose:
        Dose in mmol NH4Cl = [0.3 l/kg x bodyweight (kg)] x base excess (mmol/l);
        administer 50% of dose over at least 3 hours, then evaluate again.

        Maintenance dose:
        titrate; the normal dose is usually between 50 and 100 mg/kg/day.

         

        • The figure of 0.3 l/kg in the formula is the extracellular bicarbonate and the base excess is determined by laboratory measurement and reported with the arterial blood gases
        • Dilute to 0.2 mmol/ml (max. 0.4 mmol/ml) before administration. Maximum infusion rate 1 mmol/kg/hour.


        There is no evidence regarding the use of ammonium chloride in children for metabolic alkalosis. The dosage is determined using the weight, in the same way as for adults

    • 1 month up to 18 years
      • 75 mg/kg/day in 4 doses. Max: 6 g/day.
  • Oral
    • 1 month up to 18 years
      • 75 mg/kg/day in 4 doses. Max: 6 g/day.
