Dosages

Tetanus Post-Exposure Prophylaxis (PEP) if not or incompletely vaccinated according to (Dutch) National Vaccination Programme
  • Intramuscular
    • 0 years up to 18 years
      [3]
      • 250 - 500 IU/dose, once only.
      • Directions for administration:

        Inject slowly, deep into the muscle. The preparation should preferably be brought to body temperature before administering it.

      • If there are severe injuries, if the injury occurred > 24 hours ago and in adipose adults: 500 IU/dose as a one-time dose

        Fully vaccinated are:

        • Children up to 5 months:

          • if the mother has received maternal whooping cough vaccination (DKT) during pregnancy AND

          • there are no risk factors for insufficient transfer of antibodies from mother to child AND

          • the mother herself has ever been fully vaccinated against tetanus (according to RVP or equivalent).

        • Children 5 - 12 months who:

          • Have had primary DKTP vaccinations from RVP according to schedule: 3 doses of the 2-3-5-11 schedule or after maternal whooping cough vaccination 2 doses of the 3-5-11 schedule.

          • If necessary, in consultation with the JGZ, bring forward the  DKTP-HiB-HepB booster vaccination from the RVP, at least 5 months after the last dose of the primary series.

        • Children > 12 months who have received

          • Have received primary DKTP vaccinations from RVP according to schedule

          • If necessary, missed RVP vaccinations at 4 and 9 years of age will be caught up on after consultation with the JGZ.

        Post-exposure prophylaxis with tetanus immunoglobulin would seem to be useful up to 3 weeks after the wound.

    • 0 years up to 18 years

      • 250 IU/dose, once only. in combination with 1 dosis of tetanus toxoid (DTP, young children DPTP) in fully vaccinated patients. If not or incompletely vaccinated, catch up on the missing vaccinations. .
      • Post-exposure prophylaxis with tetanus immunoglobulin would seem to be useful up to 3 weeks after the wound.

Clinically manifest tetanus
  • Intramuscular
    • 1 month up to 18 years

      • 3.000 IU/day in 1 dose
      • Duration of treatment:

        2 days

      • Directions for administration:

        Inject slowly, deep into the muscle. The preparation should preferably be brought to body temperature before administering it.

      • Active immunization after recovery.

Renal impaiment in children > 3 months

GFR ≥10 ml/min/1.73m2: Dose adjustment not required.

GFR <10 ml/min/1.73m2: A general recommendation on dose adjustment cannot be provided.
