Pharmacokinetics in children

Children 6 - 12 years
Tmax: after an overnight fast: lisdexamfetamine approx. 1 hour, dexamphetamine approx. 3.8 hours and after a high-fat meal 4.7 hours.
Elimination: 96% via urine.

Im Rahmen einer therapeutischen Gesamtstrategie zur Behandlung von Aufmerksamkeitsdefizit-Hyperaktivitäts-Störungen (ADHS), wenn das Ansprechen auf eine zuvor erhaltene Behandlung mit Methylphenidat als klinisch unzureichend angesehen wird

Kinder unter 6 Jahren:
Darf nicht bei Kindern unter 6 Jahren angewendet werden. Sicherheit und Wirksamkeit in dieser Altersgruppe sind nicht erwiesen.

Kinder ab 6 Jahren:
Die Dosierung ist nach den therapeutischen Erfordernissen und dem Ansprechen des Patienten individuell einzustellen. Zu Beginn der Behandlung mit Lisdexamfetamin ist eine sorgfältige Dosistitration erforderlich.

Die Initialdosis beträgt 30 mg einmal täglich morgens. Wenn nach Einschätzung des Arztes eine niedrigere Anfangsdosis angemessen ist, können die Patienten die Behandlung mit einer Dosis von 20 mg einmal täglich morgens beginnen.

Die Dosis kann in ungefähr wöchentlichen Abständen in Schritten von jeweils 10 oder 20 mg erhöht werden. Elvanse ist in der niedrigsten  wirksamen  Dosierung  einzunehmen. Die  höchste  empfohlene  Dosis  beträgt  70 mg/Tag; höhere Dosen wurden nicht untersucht.

Erwachsene:

Eine Fortsetzung der Behandlung ins Erwachsenenalter kann bei Jugendlichen angemessen sein, deren Symptome bis ins Erwachsenenalter persistieren und die von der Behandlung eindeutig profitiert haben.[1]

Dosages

Attention deficit hyperactivity disorder (ADHD)
  • Oral
    • Capsule, hard
      • 6 years up to 18 years
        • Initial dose: 20 - 30 mg/day in 1 dose in the morning.
        • Maintenance dose: Increase the dose every week if needed based on efficacy and side effects with dose increments of 10-20 mg to 30 - 70 mg/day in 1 dose. Max: 70 mg/day.
        • Duration of treatment:

          Discontinue treatment if symptoms have not improved within a month after a suitable dose adjustment. Reduce the dose or discontinue treatment when paradoxical worsening of symptoms or intolerable side effects occur. Review the effectiveness with long-term use at least every year and consider trial periods without medication, preferably during the school holidays.

        • The dose should be titrated individually and the lowest possible dose should be applied.

          Lisdexamfetamine should be prescribed by a specialist in child and adolescent psychiatry or by doctors who are familiar with the use of this medicine in children and adolescents.

Side effects in children

Very common (≥ 10%): loss of appetite, growth retardation and reduced weight (especially in children). Insomnia. Headache. Upper abdominal pain (especially in younger children).

Common (1-10%): aggression (especially in young children), irritability, nervousness, anxiety, depression (especially in adolescents), tic disorder (especially in young children), affectlability. Dizziness, restlessness, tremor, sleepiness. Tachycardia, palpitations. Dyspnea. Diarrhea, constipation. Nausea, vomiting (especially in children). Skin rash (especially in younger children). Fatigue. Fever (especially in children). 

Uncommon (0.1-1%): hypersensitivity. Speech, dysphoria, euphoria, mania, dermatillomania, hallucinations. Dyskinesia, dysgeusia. Mydriase, blurred vision. Cardiomyopathy. Urticaria. Raynaud's phenomenon.

Furthermore, anaphylactic reaction, angioedema, Stevens-Johnson syndrome have been reported. Psychosis. Insult. Eosinophilic hepatitis.

Warnings & precautions in children

  • Growth retardation: Determine height, weight and appetite in children before onset and thereafter at least every 6 months. If necessary, interrupt treatment if the increase in height and weight is less than expected.
  • Blood pressure and pulse should be recorded on a percentile curve before onset, with each dose adjustment and then at least every six months.
  • Prior to prescribing document psychiatric disorders. The onset of de novo or worsening of pre-existing mental disorders should be monitored with each dose adjustment and then at least every six months and with each visit.
  • Patients also should be monitored for the risk of improper use, misuse and abuse of lisdexamphetamine.
  • Re-evaluation of usefulness should be assessed at least yearly, and periods off medication, preferably during school holidays, should be considered.

Alerts And Precautions Adults

  • Potenzial zu Missbrauch, Fehlgebrauch, Abhängigkeit und Zweckentfremdung:

    Patienten mit Drogenmissbrauch oder-abhängigkeit in der Vorgeschichte sollten Stimulanzien nur mit Vorsicht verschrieben werden

  • bei Patienten mit vorbestehenden psychotischen Störungen kann die Anwendung von Stimulanzien die Symptome von Verhaltens- und Denkstörungen verschlimmern
  • Besondere Vorsicht ist bei der Anwendung von Stimulanzien zur Behandlung von ADHS bei Patienten mit bipolaren Begleiterkrankungen geboten, da bei solchen Patienten Bedenken wegen einer möglichen Auslösung eines gemischten/manischen Schubs bestehen.
