Pharmacokinetics in children

Tetrazyklisches Antidepressivum; Antagonist an präsynaptischen α2-Adrenozeptoren, wodurch die zentrale Noradrenalin-Freisetzung erhöht wird. Zudem potenter Antagonist an 5-HT2- und 5-HT3-Rezeptoren, keine wesentliche Affinität zu 5-HT1A- und 5-HT1B-Rezeptoren. Potenter Antagonist an histaminergen H1-Rezeptoren (möglicherweise Grund für ausgeprägte sedierende Effekte). Moderater peripherer α1-adrenerger Antagonist (möglicherweise Grund für orthostatische Hypotension). [1,5]

 tmax= 2h, t1/2= 20–40h; Proteinbindung ca. 85 %, BV = 50 %

Pharmakodynamik

Extensive Metabolisierung nach oraler Verabreichung. In-vitro-Untersuchungen an humanen Lebermikrosomen zeigen, dass am Metabolismus CYP2D6, -1A2 und -3A4 beteiligt sind. Einige dieser Metabolite besitzen zwar eine pharmakologische Aktivität in vitro, diese hat aber aufgrund der geringen Blutkonzentration keine Bedeutung für die klinische Wirkung. [1,5]

Dosages

Depressive Erkrankungen / Angsterkrankungen und Phobien / Schlafstoerungen, depressive Symptome im Rahmen anderer Erkrankungen (z.B. autistische Stoerungen, Mutismus)
  • Oral
    • 8 years up to 18 years
      • Initial dose: 7.5 - 15 mg/dose in 1 - 2 doses.
      • Maintenance dose: 15 - 45 mg/dose in 1 - 2 doses. Max: 45 mg/day.
      • Dose titration:

        Aufdosierung: langsam, je nach Ansprechen um 7,5 – 15 mg/wöchentlich.

      • Applikation möglichst vor dem Schlafen/am Abend/zur Nacht. Mirtazapin sollte langsam und schrittweise bis zum gewünschten Ansprechen eindosiert werden, besonders bei Patienten, bei denen eine Hypotonie vorbesteht. Regelmäßige Kontrolluntersuchungen beachten (siehe Warnhinweise)

Renal impaiment in children > 3 months

Bei schwerer Nierenfunktionsstörung kontraindiziert.

Side effects in children

Studien zeigten eine signifikante Gewichtszunahme (≥ 7 %) bei 48,8% der mit Mirtazapin behandelten Kinder und Jugendlichen im Vergleich zu 5,7 % im Placebo-Arm. [6]

Adult Side Effects

Sehr häufig (≥ 10%): Gewichtszunahme, verstärkter Appetit, Schläfrigkeit, Sedierung, Kopfschmerz, Mundtrockenheit.

Häufig (1-10%): Anormale Träume, Angst, Verwirrtheit, Schlaflosigkeit, Lethargie, Schwindel, Tremor, orthostatische Hypotension, Übelkeit, Erbrechen, Obstipation, Diarrhö, Exantheme, Arthralgie, Myalgie, Rückenschmerzen, periphere Ödeme, Erschöpfung. [1]

Contra-indications in adults

Absolute bzw. relative Anwendungseinschränkungen:

  • absolut
    • Überempfindlichkeit gegen den Wirkstoff oder anderen Bestandteilen des Fertigarzneimittels
    • Kombination mit MAO-Hemmern (freies Intervall 2 Wochen)
  • relativ (nur nach strenger Nutzen-Risiko-Abwägung und entsprechenden Vorsichtsmaßnahmen)
    • Kinder <18 Jahre
    • akute Intoxikationen mit psychotropen Wirkstoffen
    • hirnorganischem Psychosyndrom
    • schwere Leber- und Nierenfunktionsstörungen
    • erhöhte Krampfbereitschaft
    • (Hypo-)Manie
    • Herz-Kreislauf-Erkrankungen (z. B. Hypotonie, Erregungsleitungsstörungen)
    • akutem Engwinkelglaukom und erhöhtem Augeninnendruck
    • Diabetes mellitus
    • Suizidalität
    • gleichzeitige Anwendung mit Arzneimitteln, die eine Hyponatriämie verursachen oder die QT-Zeit verlängern oder mit anderen serotonergen Substanzen (u.a. SSRIs, Johanniskraut, Lithium)
