With impaired renal function, the AUC of olmesartan increases. This increases the risk of adverse reactions.
Clinical implications:
The main problem with overdose is hypotension. Due to inhibition of the RAAS, some decline in renal function is possible. On the other hand, AT-II antagonists have a beneficial effect on renal function by reducing proteinuria and increasing urinary nitrate/nitrite excretion.
