Adult Side Effects
De belangrijkste bijwerkingen zijn tachycardie, hypotensie, angina pectoris, hoofdpijn en misselijkheid.
Kan een aanval van acute porfyrie uitlokken.
Bij minder dan 1% van de patiënten: reversibele leverbeschadiging.
Zelden zijn gemeld bloedbeeldafwijkingen, zoals anemie, leukopenie en trombocytopenie, psychiatrische stoornissen, exantheem en koorts. Na het ontstaan van deze verschijnselen dient de toediening te worden gestaakt. Ook zelden zijn gemeld lupus erythematodes-achtige syndromen en een reumatoïde artritis-achtig beeld na langdurig gebruik, seksuele dysfunctie, perifere neuritis (wordt tegengegaan door pyridoxine), glomerulonefritis en vasculitis.
Verder zijn gemeld tachycardie (wordt tegengegaan door een β-blokker), natrium- en waterretentie (te voorkomen door een diureticum), hoofdpijn, blozen, duizeligheid, hartkloppingen, maagdarmstoornissen, nasale congestie, conjunctivale infectie, oedeem, tremor, spierkramp, paresthesie, urticaria, perifere neuropathie, hypotensie en angineuze klachten. Deze bijwerkingen verminderen of verdwijnen veelal bij voortzetten van de behandeling of bij verlaging van de dosering.
Langdurig gebruik (meer dan 6 maanden) kan, vooral bij doses van meer dan 100 mg per dag, een op systemische lupus erythematodes (SLE) lijkend beeld veroorzaken. In de lichte vorm lijkt het op chronische polyartritis, in ernstige vorm lijkt het syndroom op acuut SLE. Bij vrouwen en bij langzame acetyleerders is de incidentie hoger. Na het ontstaan van deze verschijnselen dient de toediening te worden gestaakt. De verschijnselen zijn gewoonlijk reversibel. Aanbevolen wordt bij langdurige therapie met tussenpozen van ong. 6 maanden de antinucleaire factoren (ANF) te bepalen.

Contra-indications in adults
De belangrijkste contra-indicaties zijn aneurysma aortae dissecans, hartfalen met hoog hartminuutvolume en myocardinfarct.
Toepassing bij acute porfyrie wordt ontraden omdat het hierbij mogelijk onveilig is.
Voorzichtigheid is geboden angina pectoris omdat dihydralazine tachycardie kan veroorzaken waardoor angina pectoris kan verergeren, en bij porphyria cutanea tarda omdat dit kan verergeren.

Warnings & precautions in children
Voorzichtigheid is geboden bij coronair lijden (tenzij in combinatie met een β-blokker), bij onbehandelde of niet adequaat behandelde angina pectoris, bij een recent myocardinfarct, bij cerebrovasculaire aandoeningen en bij leverfunctiestoornis. Bij verminderde nierfunctie (creatinineklaring groter dan 10 ml/min) is aanpassing van de dosering niet noodzakelijk.Het risico van lupus erythematodes-achtige syndromen neemt toe bij hogere doseringen agv acetylator type (> 1,3 mg/kg/dag) en de duur van de therapie (> 6 mnd.).
Overweeg bij hogere doseringen het acetylator type te bepalen.
